Works collaboratively with all members of the health care team to coordinate, facilitate, and manage the clinical, financial, and patient flow activities of assigned patients. Has the authority and responsibility to address all related patient care issues. Performs a wide variety of patient care activities to include screening assessment, monitoring of patient care activities, development of a plan to address physical and psychosocial needs, utilization management functions, review care paths and update with the health care team as needed, coordinate services and discharge planning, and coordinate patient/family teaching. Demonstrates clinical expertise as well as an understanding of systems theory and its impact upon the change process in order to provide a complex level of intervention.
A Bachelor’s degree in Nursing or related health care field with 3 years of current, relevant clinical experience of which 2 years is in pediatrics is required; Case Management experience is strongly preferred. A Masters in Nursing or related health care field is preferred. Currently licensed as a Registered Nurse in the State of Florida. Must understand systems and change process. Must have a working knowledge of health care financial systems. Must have excellent decision-making abilities and problem-solving skills. Must have excellent interpersonal skills with the ability to work with diverse individuals, excellent communication skills, and good writing and computer skills. Must be able to work well under pressure and manage stress effectively. Must be knowledgeable about and intervene effectively with the unique age-specific needs of the assigned patients and families as defined in the Patient Contact section of the position description.
